School Immunization Requirements

Ensuring that children are up-to-date on their immunizations is a vital step in protecting the health of students and the broader community. Below is essential information about the immunization requirements for school enrollment.

Required Immunizations for School Enrollment

To attend school, children must have received the following vaccinations as recommended for their age group:

  • Diphtheria, Tetanus, and Pertussis (DTaP, Tdap)
  • Polio (IPV)
  • Measles, Mumps, and Rubella (MMR)
  • Hepatitis B
  • Varicella (Chickenpox)
  • Meningococcal (MCV4) – required for middle and high school students in most cases.

Each state may have specific schedules and dosage requirements. Please consult with your healthcare provider or school for detailed guidance. View the Missouri immunization requirements here: Missouri School Immunization Requirements

Documentation Required

Parents or guardians must provide proof of immunization before their child begins school. Accepted documentation includes:

  • An official immunization record from a healthcare provider.
  • A copy of the state’s immunization registry report.
  • A completed and signed Certificate of Immunization form.

Exemptions

While immunization is critical for public health, some exemptions are permitted:

  • Medical Exemptions: Requires documentation from a licensed healthcare provider stating the medical reason for exemption.
  • Religious or Personal Beliefs Exemptions: Permitted in some states with a completed exemption form.

Please note that exempt students may be excluded from school during outbreaks of vaccine-preventable diseases.

Immunization Deadlines

To avoid delays in enrollment, immunization records must be submitted by the following deadlines:

  • Kindergarten Enrollment: Before the first day of school.
  • Middle and High School Enrollment: Updates may be required before advancing grades.

Where to Get Immunizations

Vaccinations are available through:

  • Primary Care Providers
  • Local Health Departments
  • Community Clinics

Many health departments offer low-cost or free vaccinations for families in need.
