What Is HIV Case Management?

HIV Case Management provides personalized support to individuals diagnosed with HIV, assisting them in accessing essential services and maintaining a healthy, fulfilling life.

Services We Provide:

• Personalized Care Plans: Tailored to address each client’s unique needs and goals.
• Medical Access: Assistance with scheduling appointments, coordinating care with providers, and accessing medications.

Who Can Benefit?

Anyone living with HIV who resides in Northwest Missouri is eligible for this program. The only criteria for participation are:

• A diagnosis of HIV+.
• Residency in Missouri’s northwest region.
